4J Insurance Brokerage is a veteran-owned commercial insurance brokerage headquartered at 2591 Dallas Pkwy, Suite 300, Frisco, TX 75034. We are licensed in Texas (Agency License #2911773) and Oklahoma (Agency License #3003920647), among other states.

Information you give us. When you request a quote, schedule a consultation, submit a form, or email us, you may provide your name, business name, job title, email address, phone number, mailing address, and details about your business and its risk exposures — including operations, payroll, revenue, property, vehicles, prior claims, and current coverage.
Employee benefits information. If we place group health or employee benefits coverage for you, we may receive census data about your employees, including names, dates of birth, ZIP codes, and dependent status. Where we receive protected health information, we handle it in accordance with HIPAA.
Information collected automatically. Our website runs on HubSpot. When you visit, we and our service providers may automatically collect your IP address, browser type, device type, referring page, pages viewed, and time spent, using cookies and similar technologies.
How we use your information
We use it to:
- Prepare quotes and applications, and submit them to insurance carriers on your behalf
- Place, service, renew, and modify your insurance coverage
